in 19 years of steelhead fishing, i've had 2 actually swallow the hook -- one was on eggs 3 years ago, the other on soft plastics 3 weeks ago.
salmon on the other hand, if i plan on releasing some fish, i always try to get a quick hookset because kings and silvers will usually swallow the hook if you let them chomp on eggs for too long, and be bleeding to death by the time i get them to hand.
